Isometrics for Blood Pressure
Four two-minute holds — the strongest effect among all exercise types
Who it’s for
High blood pressure, cleared by a doctor for moderate exercise.
How it works
Four two-minute holds at roughly 30% of maximum voluntary effort, with one minute of rest between sets. In a network meta-analysis of 270 studies and nearly 16,000 participants, isometric training lowered systolic blood pressure by 8.2 mmHg — more than aerobic (4.5), strength (4.6), or interval training (4.1). A fair caveat: isometric studies are fewer and lower quality, so this is a supplement to walking and strength work, not a replacement.
Weekly layout
How load increases
Eight weeks without changes — the effect accumulates on its own. Only the quality of the hold improves: steady breathing, no breath-holding. The Valsalva maneuver is strictly prohibited: straining to failure can spike arterial pressure into triple digits even in healthy individuals.
Deload
Not required.
