Reduced bone density, osteopenia, or osteoporosis without recent fractures.

How it works

Bone responds to the magnitude and rate of strain, not high volume — which is why this program uses low sets and heavy weights. Five sets of five reps in the squat, overhead press, and deadlift, plus soft landings. In the LIFTMOR trial, this exact protocol twice a week for 30 minutes yielded a +2.9% lumbar spine density increase over eight months compared to −1.2% in the control group, with just one mild adverse event per 49 women. Swimming and cycling do not affect bone density at all: no impact means no signal.

How load increases

The first four weeks focus purely on technique with light weight. After that, 5 sets of 5 reps with a weight that allows you to maintain a neutral spine on every rep, adding weight once a week. The spine must stay neutral at all times: loaded spinal flexion, not the weight itself, is linked to new vertebral fractures.

Deload

Every eighth week: cut the sets in half, keep the weight the same.
